You have the right under Ohio law to designate one person to have authority to make the decision of what to do with your body at the time of death. Why would you even need such a document?
- Your children disagree among themselves about the “proper” way to take care of you
- You are estranged from your family but have close friends who you trust;
- You are gay/lesbian and want your partner to manage your memorial and affairs after your death; or
- Your sister is living in China and can’t be reached quickly but you trust your best friend.
Copies of this form for yourself and family will be available at the annual meeting, or you can call the office. Please fill one out now and keep it with your final papers.
